This is a drug free workplace. All applicants and employees are subject to substance abuse testing as a condition of hiring and continued employment.

It is understood and agreed that any misrepresentation by me in this application will be sufficient cause for cancellation of this application and/or termination of employment from New Madrid County Ambulance District (Employer) if I have been employed. Furthermore, I understand that just as I am free to resign at any time, New Madrid County Ambulance District reserves the right to terminate my employment at any time, with or without cause and without prior notice.

I understand that as a condition of continuous employment, I am subject to taking drug and alcohol tests, when given in accordance with company policies and procedures and that refusal to take drug and alcohol tests will be grounds for immediate termination of employment.

I give New Madrid County Ambulance District the right to investigate all references and to secure additional information about me, if job related. I hereby release from liability New Madrid County Ambulance District and its representatives for seeking such information, and all other persons, corporations or organizations for furnishing such information.
